Pets must be current on vaccines and flea tick control for the protection of all our patients. If vaccines/frontline were done elsewhere, printed verification is required at boarding check-in.
Pets will be fed twice daily unless otherwise agreed upon. Fresh water is available at all times. Our boarding facilities are properly cleaned and sanitized daily.
You may wish to have your pet bathed during or before boarding release. Our basic boarding spa services include bathing and hydration with Iv San Bernard products and are a comforting and refreshing addition to your pet's stay with us. Please choose from the following:
* I am aware that all pets must be current on vaccinations and flea/tick control. If these are not current, they will be done by the staff at AHS upon admission.
* I understand that should my pet be left at AHS for greater than 7 days after the date I have specified for pickup without contacting AHS, that abandonment proceedings will begin.
* I understand that should my pet need emergency care, AHS will make every attempt to reach me or my emergency contacts. Please indicate below how you would like us to proceed with treatment for your pet.
In the event that I am unavailable, I elect complete medical care for my pet and will assume all responsibility for charges incurred.
In the event that I am unavailable, I elect minimal treatment for my pet to prevent life-threatening concerns and will assume all responsibility for charges incurred.
I decline treatment for my pet without my permission. I understand that if I am unavailable and my pet's life is threatened, no treatment shall be done; I waive all responsibility of the emergency contact and AHS.
